Qualifications

  • Two (2) years’ experience providing related services to families and individuals; one year with a Master’s Degree.
  • Knowledge of community-based services, public assistance, and demonstrated skills in accessing these services.
  • Must have a valid driver’s license and be willing to travel.

Responsibilities

  • Conduct comprehensive assessments to determine whether individuals and families require specialized services, such as mental health or substance abuse treatment, to achieve stability. Help identify unaddressed needs and guide employment planning, ensuring that services are tailored to the specific circumstances of each family.
  • Assist families with complex care needs in navigating State and Federal benefits programs to ensure they receive all benefits to which they are entitled and remain in compliance with program requirements, preventing families from losing access to essential supports due to misunderstandings or missed requirements.
  • Make referrals to in-house licensed mental health counselors when appropriate or connect customers to Single Point of Access and Children’s Single Point of Access for timely and appropriate mental health and behavioral services. Assist in applying for benefits such as SS or help with obtaining necessary documentation.
  • Work as a liaison between families and community service providers to ensure treatment that aligns with each family’s goals for stabilization and well-being, including identifying specific treatment, rehabilitation, and support needs and linking families to appropriate services while monitoring their progress.
  • Monitoring of families’ progress to ensure they remain on track with their stabilization plans. Provide supportive services to help families attend appointments and follow through on service plans, preventing disengagement and promoting long-term stability. Collaborate with the in-house employment service staff, community partners as well as county district employment staff as appropriate to develop suitable employment plans.
  • Ensure comprehensive access to all SCAP’s services, working with agency colleagues and community partners to fully integrate, streamline and coordinate services.
  • Conduct advocacy on behalf of households in a solution-focused manner and represent SCAP in a positive, professional manner.
Qualifications

Requirements

  • Bachelor’s Degree in social work, human services or related field, Master’s Degree preferred.
  • Two (2) years’ experience providing related services to families and individuals; one year with a Master’s Degree.
  • Knowledge of community-based services, public assistance and other community resources and demonstrated skills in accessing these services appropriately. Previous experience working in the field of mental health, substance abuse and/or housing support. Must have the ability to learn quickly the resources available and be able to appropriately refer in-house to ensure full access to SCAP’s services.
  • Must have demonstrated written and verbal communication skills and employ appropriate boundaries, representing SCAP in a professional manner.
  • Must demonstrate ability to perform job responsibilities independently.
  • Must have a valid driver’s license, have regular access to a car and be willing to travel to off-site locations.
  • Demonstrated computer expertise and be able to stay up to date on software used in best practices.
  • Background check is required.
  • Knowledge and understanding of issues related to individuals and families in poverty. Ability to build upon the strengths of those we serve with diverse racial, cultural, social, educational and economic backgrounds, including high risk populations, to provide high-level services and lasting solutions to fit each individual’s and family’s needs.
