Job description

Join to apply for the Family Case Manager-NG532501 role at ICL.

Responsibilities include:

  1. Traveling to consumers' residences to provide supportive services and counseling to help modify attitudes and behaviors.
  2. Securing and evaluating information to develop and maintain consumer treatment plans and case records.
  3. Providing supportive counseling and developing resource information for consumers.
  4. Using cognitive-behavioral techniques to meet service plan goals and facilitate discharge planning.
  5. Explaining available services and instructing consumers in daily living, socialization, parenting, and conflict resolution skills.
  6. Performing crisis assessments and interventions, and referring consumers to community resources.
  7. Handling incident reports, conducting residence safety inspections, and executing emergency plans.
  8. Supporting new personnel, attending meetings and training, and advocating for consumers.
  9. Assisting consumers in vocational training opportunities and possibly providing substance abuse services.

Minimum Education and Experience:

  • Master's degree in a qualifying field with 1 year of experience, OR
  • Bachelor's degree in a qualifying field with 2 years of experience, OR
  • Credentialed Alcoholism and Substance Abuse Counselor (CASAC) with 2 years of experience, OR
  • Bachelor's degree or higher with 3 years of experience or 2 years serving the SMI or SED population.

Experience in providing services to individuals with Serious Mental Illness, developmental disabilities, or substance abuse, and linking them to services is preferred.

Preference given to applicants with experience with young adults and families.

Required skills include effective communication, case management, documentation, cultural sensitivity, and the ability to work independently and collaboratively.
