Job Description


PsychPros is currently hiring for a full time, temp to hire case management position in Butler County, Ohio. In this role, you will be working with families within the community involved with Children’s Protective Services with the goal of family preservation. Clients will have mental health or behavioral health conditions.

Requirements-

  • Bachelor's degree in social services field
  • Reliable transportation
  • Ability to work flexible schedule to meet clients’ needs
  • One year minimum experience providing case management to families or children

Job Duties-
  • Assess and address the MH needs of child welfare-involved families, help families gain access to needed medical, social, educational, and other services through natural supports, (e.g., family or friends, community resources and specialized mental health treatment, rehabilitation, and support services), in order to achieve specific outcomes of family reunification/stabilization, independent living, vocational/educational participation, adequate social supports and reduced hospitalization.
  • The case manager serves as a key member of the treatment/service team, assuring often complex services produce positive outcomes.
  • Develop and implement proactive intervention strategies.
  • Follow the conjointly developed service plan pursuing all of the objectives developed.
  • Work with the family to alter the service plan to meet the family’s changing needs and utilizing the new strengths.
  • Be the go-to MH person for the family and other systems they are involved with.
  • Work towards smooth coordination of service delivery among systems.
  • Evaluate all services received by the family.
  • Manage crises.
