Qualifications

Position Summary:The Family Care Coordinator Associate is responsible for case management including home visits and classroom support, as well as program support including recruitment and transportation coordination. This role carries a smaller caseload and participates in recruitment and enrollment activities, ongoing case management, classroom support, and connecting children and families with resources and services including.Responsibilities:Participate in recruitment and enrollment activities.Arrange and promote recruitment activities including presentations to community partnersComplete enrollment paperwork including designated screening formsObserve the parent-child interaction during intake through enrollment phase.Provide ongoing case management.Provide home visits with caregivers at least monthly to address family needs, parenting skills and behavior planning consistent with the classroom and Family Care Plan.Coach caregivers on responsive parenting, child development, and social-emotional learning.Collaborate with caregivers, classroom staff and Mental Health Consultant.Arrange multidisciplinary meetings every 90 days and Maintain consistent communication with families to monitor progress and provide ongoing support.Monitor family’s need for ECLIPSE transportation, arrange for bus service if needed and document in child record.Support behavior strategies provided by the MHC within classroom and home settings.Provide classroom observations and support; follow up with classroom staff regarding interventions, children’s progress or concerns, and family needs.Make referrals to community services whenever necessary.Maintain a caseload of 10-16 families.Coordinate ECLIPSE bus services for the program.Communicate program needs with People for People for transportation servicesMaintain ongoing communication with childcare staff, Family Support Staff and Program Managers for changes and updatesMaintain bus schedule/route information and distribute, as requiredComplete all documentation according to agency policy and required by the contract.Document home visits, community meetings, MDT meetings and other contacts on calendar dailyComplete documentation within 48 hours of scheduled visitsKeep ELMS/Documentation tracker updated, as requiredAttend and plan for transition meetings in conjunction with Mental Health Consultant upon family’s approach and exit from ECLIPSEDocument all phone calls, case notes, contacts with allied service providers and other treatment information in the electronic record according to the ECLIPSE policies and procedures.Maintain communication with assigned social workers and referral sources regarding progress and concerns.Follow up with classroom staff to make sure objective assessments are completed and scored on time.Meet productivity standards of 50% of time in direct face-to-face services including home visits, multidisciplinary team meetings, case management, classroom observations, etc.Maintain confidentiality according to HIPAA and FERPA in all aspects of the programActively participate in team operations and supervision.Communicate directly with supervisor for any questions or concerns regarding program design/implementation, job requirements, behavior interventions, training or general program needs.Staff cases that require team coordination or therapeutic input with Mental Health Consultant and/or Program Manager, as neededOrganize and participate in family events, support groups, and parent meetingsPerform other duties as assignedJob Requirements:The following requirements are those that are normally required for performance of this position. In addition, approximately 40% of the time may include lifting and carrying children, moving quickly around the room to maintain appropriate supervision and safety and sitting on the floor or in small chairs with the group.Non-physical Requirements:EducationAssociate’s degree in early childhood education, human development, psychology, education, or closely related field preferred, or high school diploma or GED and approved professional development plan to obtain the associate’s degree within 5 years of hireExperience1 year of experience working with parents/caregivers and/or at-risk childrenSpecial SkillsPossess behavioral management skillsPossess good verbal and written communication skillsKnowledge of child abuse and neglect issues, home visiting, parenting skills training, mental health and community resources.Ability to work independently and as a team memberAbility to represent the agency in a professional manner within the communityAbility to work in a collaborative manner with referral sources and team membersComplete accurate paperwork in a timely mannerBasic computer proficiencyBilingual/Biliterate in English & Spanish requiredLicensure, Registration, CertificationRegistered Agency Affiliated Counselor credentialWilling to obtain endorsement through Center for Early Relational Health as Infant Family Associate or Early Childhood Family Associate, or higherComplete Mandated Reporter training upon hireFirst Aid/CPR, Bloodborne Pathogens and HIV training upon hireValid driver’s license and minimum required liability insurance for WA State.Must be deemed insurable as determined by Catholic Charities’ liability insurance providerEmployment Conditional Upon:Being cleared by criminal background check and fingerprinting when requiredWork Schedule: Monday-Friday, 8:00am - 5:00pmWage Range: $22.00 - $25.50 per hour, depending experienceBenefits:13 paid holidays, 12 days of vacation, 12 days of sick leave per yearHealth insurance including medical & prescription coverage, with optional dental and vision insurance plans.
