Job summary

Merakey is seeking a Family Based Case Manager to join our Child and Family Services team in Kennett Square, PA, serving Chester County. You will provide family-based treatment services to support children with mental health and emotional needs, and you will coordinate with schools and healthcare providers.

This role requires a Bachelor's degree in a related field and at least one year of experience; valid PA driver's license and reliable transportation are needed.

Qualifications

• Bachelor's degree in psychology, social work, or related discipline.
• Minimum of one year of experience working with children or adolescents.
• Valid Pennsylvania driver's license and access to reliable transportation.

Responsibilities

• Provide family-based treatment services and skills training.
• Maintain regular communication with families, schools, physicians, and other service providers involved with the children and family.
• Travel to clients' homes and community resources to deliver home-based services.
• Work flexible hours including evenings and weekends.
