Oregon Child Development Coalition (OCDC) is seeking a Program Case Manager to provide direct health and social services to children and families, conduct home visits, enrollments, and coordinate with Head Start requirements.

The role emphasizes community referrals, data entry in electronic tracking, safeguarding confidentiality, and collaboration with family and health services to develop Care Plans.

Qualificações

• High school diploma or GED required and select credential in social work or related field.
• Must complete Family Advocate Certificate Level 1 within 12 months of hire.
• 1–2 years in health/social services or related administrative/customer support duties considered.

Responsabilidades

• Provide direct health and social services to children and families including enrollment, health screenings, and home visits.
• Refer families to community agencies for emergency assistance, education, job training, health, and dental services.
• Maintain records, enter data, and compile monthly reports for supervisor.
• Advocate for Head Start families and support parent involvement and community advocacy.
