Job summary

Abraxas Youth and Family Services is seeking a Family Advocate to support planning and discharge processes for children, coordinating with families, agencies, and providers to ensure best interests are met. You will assist in aftercare/discharge planning and case management, while promoting collaboration and treatment progress.

Qualifications

• Minimum of two years’ experience in social services or related field.
• Background/experience with trauma-informed care, family dynamics, conflict resolution, and cultural diversity.

Responsibilities

• Coordinate and implement case management activities with family / discharge planning
• Provide indirect supervision of clinical / case management positions to direct and monitor clinical services related to family and discharge planning
• Monitor level of family involvement/engagement and assist treatment team in making recommendations on clients’ best interest related to family support
• Assist in the development and implementation of individualized treatment plans
• Assist clients in understanding established treatment goals, service plans, and rights and expectations
• Assist clients in knowing available resources and supports at the program and in the community
• Provide or assist in group counseling sessions and facilitate understanding of presented information
• Document services provided and communicate with external agencies involved in client treatment plans
• Write reports accurately and support advocacy for client and family involvement
• Attend court hearings, advocacy meetings, and treatment planning meetings as necessary
• Communicate effectively with juvenile probation officers/case workers and involve them in the treatment process
• Maintain regular contact with parents/legal guardians and update them on client progress
• Provide recommendations on continuing care planning and referrals
• Assist in services related to Specialized Treatment settings to ensure standards are met
• Support the Abraxas philosophy and Seven Key Principles of care
• Demonstrate Safe Crisis Management techniques and skills

What You’ll Do

As a Family Advocate, you will help ensure there is a collaborative process with families, placing agencies and providers, with the specific focus on the best interest of each child. Planning and process will focus on aftercare/discharge planning and case management support services to each child.

• Assist in the development and implementation of individualized treatment plans
• Assist clients in the understanding of established treatment goals, service plans, and rights and expectations
• Assist clients in knowing what resources and supports are available to them at the program and in the community
• Provide or assist in group counseling sessions, facilitating and helping clients understand information being presented
• Participate in and documents services provided, as well as communication with external agencies/services involved in supporting client treatment plans
• Write reports accurately and support advocacy for client and family involvement
• Attend court hearings, advocacy meetings, and treatment planning meetings as necessary
• Communicate effectively with juvenile probation officers/case workers, including them in the treatment process and updating them on client progress related to responsibilities
• Maintain regular contact and communicate effectively with parents/legal guardians, including them in the treatment process and updating them on client progress
• Provide recommendations on continuing care planning by making contacts to appropriate referrals, communicating with the treatment team and creating the plan for reintegration
• Assist in services related to Specialized Treatment settings to ensure standards are met
• Support the Abraxas philosophy and mission and promotes the Seven Key Principles of care.
• Demonstrate appropriate use of Safe Crisis Management techniques and skills.

Qualifications

Education and Experience Preferred:

• Minimum of two years’ experience in social services or related field
• Background/experience with trauma-informed care, family dynamics, conflict resolution, and cultural diversity

Other Qualifications:

• At least twenty-one (21) years of age
• Must have a valid Driver’s License
• Physical exam that includes TB and drug testing.
• Must pass both federal and state background checks which screen for crimes including, but not limited to drug related crimes, violent crimes, sexual abuse, domestic violence, child abuse, and repeated misdemeanors.
• Satisfactory completion of background screening and applicable pre-employment checks, including but not limited to employment and/or personal reference and driving records.
• Ability to work with computers and the necessary software typically used by the department.

Schedule

• Full-Time
• Monday-Friday, 9am-5pm
