Job summary

Mountain Valley Hospice is seeking a Facility Hospice Care Coordinator – LPN to support patients in SNFs and ALFs. This hybrid role combines nursing care with proactive communication to keep patients, families, facility staff, and the hospice team informed and connected.

You will round in facilities, coordinate care, provide nursing visits, reinforce hospice education, and promptly communicate changes to the RN Case Manager. Strong communication and multi-site coordination are essential.

Qualifications

• Active North Carolina LPN license required.
• Hospice, home health, SNF or ALF experience preferred.
• Excellent interpersonal and patient-centered communication.
• Strong organization with multi-facility coordination.
• Reliable transportation and ability to travel across service area.

Responsibilities

• Coordinate care by rounding in SNFs and ALFs and participate in care plan meetings.
• Provide proactive communication with patients and families after changes in condition.
• Complete PRN and supplemental LPN visits and reinforce hospice education.
• Identify concerns and relay them to the appropriate clinical team member.

Join Our Team:

Mountain Valley is seeking a Facility Hospice Care Coordinator – LPN to support hospice patients residing in skilled nursing facilities (SNFs) and assisted living facilities (ALFs). This hybrid position combines nursing care with proactive communication and care coordination to help keep patients, families, facility staff, and the Mountain Valley hospice team informed and connected.

What You'll Do:

• Coordinate Care: Round routinely in SNFs and ALFs, build relationships with facility staff, and participate in facility care plan meetings as appropriate.
• Keep Families Connected: Provide proactive communication and follow-up with patients and families, particularly following changes in condition or significant events.
• Provide Nursing Support: Complete PRN and supplemental LPN visits, reinforce hospice education, and recognize and communicate changes in patient condition to the RN Case Manager.
• Support the Care Team: Identify communication or service concerns and ensure they are communicated to the appropriate clinical team member or leader for follow-up.

Your Schedule:

• Monday - Friday, 8 AM - 5 PM.
• Hybrid schedule with travel to skilled nursing and assisted living facilities.

Qualifications

• Valid North Carolina Licensed Practical Nurse (LPN) license.
• Hospice, home health, skilled nursing, or assisted living experience preferred.
• Exceptional interpersonal and communication skills with a compassionate and patient-centered approach.
• Strong organization and follow-through with the ability to manage needs across multiple facilities.
• Ability to work independently and collaboratively as part of the interdisciplinary hospice team.
• Reliable transportation and ability to travel throughout the assigned service area.
