Centra Healthcare Solutions Inc is seeking an experienced or newly graduated Physical Therapist (PT) to join their SNF/LTC team. This direct hire (perm) position features a days schedule and requires adherence to the organization’s vision, values, mission and goals.

The candidate must be licensed as a Registered Physical Therapist in the state, have graduated from an accredited program, and hold CPR certification.

Qualificações

• Must have graduated from an accredited school.
• Current state license as a Registered Physical Therapist (PT).
• Current CPR certification.

Responsabilidades

• Develop a plan of care for each physical therapy patient.
• Provide skilled physical therapy services / interventions according to physician orders.
• Assess patient needs, plan, evaluate and modify care to meet PT goals.
• Collaborate with all disciplines to plan and evaluate team goals for each patient.
