Job summary

Centra Healthcare Solutions, Inc. is seeking an experienced or newly graduated Physical Therapist to join a SNF/LTC team in a direct hire role. The position offers a perm arrangement and day schedule.

The PT will provide skilled physical therapy services per physician orders, develop a plan of care for each patient, and collaborate with the care team to meet goals and ensure high-quality patient outcomes.

Qualifications

• Must have graduated from an accredited school.
• Current state license as a Registered Physical Therapist (PT) or actively pursuing licensure.
• Current CPR certification.
• Develop a plan of care for each physical therapy patient.
• Provide skilled physical therapy services per physician orders.
• Assess patient needs, plan, evaluate and modify care to meet goals of physical therapy interventions.
• Collaborates with all disciplines to plan and evaluate team goals for each patient.

Responsibilities

• Develop and implement individualized physical therapy plans of care.
• Deliver skilled PT services according to physician orders.
• Assess and document patient progress and modify treatment plans as needed.
• Collaborate with nursing and other disciplines to achieve patient goals.
• Ensure compliance with state licensing and CPR requirements.
• Participate in discharge planning and care coordination.
