Job summary

The role requires a current Physical Therapist license, CPR certification, and a 4-year degree. Strong communication, documentation, and patient education skills are essential; prior long-term care

Qualifications

• Graduate of an approved Physical Therapy curriculum.
• Licensed to practice as a Physical Therapist with CPR certification.
• Knowledge of Medicare and payor sources.
• Strong communication and patient education skills.
• Ability to manage multiple tasks and maintain professionalism.

Responsibilities

• Makes evaluations initiated per M.D. orders and payer guidelines.
• Develops treatment plans and provides care for best functional outcome.
• Educates staff and third-party payers on benefits and outcomes.
• Completes documentation including evaluations and progress notes.
• Screens individuals who may benefit from therapy.
