Job summary

Compass Healthcare Consulting & Placement is conducting a search for an experienced LPN for an Assisted Living Facility in Staten Island, NY. Shifts are 11:30AM - 8:00PM Monday through Friday, with a competitive salary and excellent benefits. The role requires NY LPN license and 1–2 years of relevant experience. Responsibilities include supervising aides, medication administration, and care planning.

Qualifications

• Graduate of an accredited practical nursing program.
• Valid New York State License as a Licensed Practical Nurse (LPN).
• Minimum of 1-2 years of experience in community nursing, acute care, or adult care facilities.
• Supervisory experience and medication management preferred.

Responsibilities

• Provide supervision and direction to Home Health Aides.
• Deliver nursing care in accordance with agency policies and procedures.
• Participate in incident reporting and follow-up actions.
• Complete, maintain and submit accurate clinical records and prepare oral reports.
• Assist in preparing and evaluating nursing care plans.
• Observe, document, and report changes in residents’ conditions.
• Perform nursing procedures, including medication administration and treatments.
• Manage medication for residents, including changes, new prescriptions, and refills.
• Assist Nursing management with assessments and reassessments of residents to determine health needs.
• Foster a positive working environment, promoting teamwork and staff development.
• Address employee concerns and ensure staff adhere to policies.
• Ensure a safe and healthy living environment for all residents.
• Undertake resident/family teaching for self-care and continuity of care.
