Job summary

Assurance Home Care in California is seeking an Executive Director/Administrator (RN) to lead the licensed Home Health Agency. You will oversee regulatory compliance, clinical operations, and financial performance, guiding the team through surveys, accreditation visits, and CMS audits.

The successful candidate will have 3–5 years of CA home health operations leadership, an active RN license, and an eligible Home Health Administrator certificate or ability to obtain within 90 days.

Qualifications

• Bachelor’s degree in Nursing required.

• Home Health Administrator certificate or ability to obtain within 90 days of hire.
• Minimum 3–5 years of home health operations management experience in California.

Responsibilities

• Oversee regulatory and clinical compliance (California focus).
• Ensure compliance with California Health & Safety Code, Title 22, and Medicare CoPs.
• Lead governance liaison and maintain valid California home health license.
• Oversee OASIS accuracy, Plan of Care integrity, and clinical operations.
• Direct daily operations: intake, scheduling, billing and HR.
• Develop policies compliant with California labor laws (meal breaks, time pay).
• Manage branch growth strategy and market share in CA region.
• Oversee P&L, budgeting, and revenue cycle management.
• Monitor Medicare margins and payor mix for California plans.
• Supervise Clinical Manager, Therapy Supervisor, and admin leads.

If Yes, Then As An Executive Director/Administrator You Will

The Executive Director/Administrator is responsible for the overall strategic, clinical, financial, and operational management of the licensed Home Health Agency (HHA). This role ensures compliance with all California state regulations, federal Conditions of Participation (CoPs), and CMS standards.

• Regulatory & Clinical Compliance (California Focus)
• Maintains compliance with California Health & Safety Code, Title 22, and Medicare CoPs.
• Acts as the governing body liaison; ensures the agency holds a valid California home health license.
• Reviews clinical operations, OASIS accuracy, and Plan of Care (POC) integrity.
• Leads all state surveys, CHAP/Joint Commission accreditation visits, and CMS audits.
• Operational Management
• Directs daily operations including intake, scheduling, billing, and HR.
• Develops and implements policies specific to California labor laws (e.g., reporting time pay, meal breaks) as they apply to field staff.
• Manages branch growth strategy, including market share in [Specific CA region].
• Financial Oversight
• Manages P&L, budgeting, and revenue cycle management.
• Monitors Medicare margin and payor mix specific to California’s managed Medicare plans.
• Leadership & Staff Management
• Supervises Clinical Manager (often an RN), Therapy Supervisor, and administrative leads.
• (If non-clinical) Must demonstrate ability to effectively supervise RNs and clinical staff without practicing nursing.

Must Haves

• Education: Bachelor’s degree in Nursing.
• Experience: Minimum 3-5 years of home health operations management experience in California.
• Administrator Certification: Active Home Health Administrator certificate (from an approved California provider) OR ability to obtain within 90 days of hire.
• License: Current, active, unencumbered Registered Nursing license issued by the California Board of Registered Nursing (BRN).
• Experience: Prior experience as a Director of Patient Care Services (DPCS) or Clinical Manager in a Medicare-certified home health agency.
