About MyPlace Health

myPlace Health was founded in 2021 by mission-driven healthcare leaders and organizations that are committed to drastically improving health outcomes, quality and experience for vulnerable older adults and frail seniors. We are an integrated care delivery organization that specializes in providing personalized care and coverage to older adults who wish to remain safely living in their homes and communities for as long as possible, rather than living in a nursing facility.

Our mission is simple: to enable seniors to live the independent lives they deserve. The primary ways in which we pursue our mission are through capitated arrangements with government payers, such as Medicare and Medicaid under the PACE (Program of All-Inclusive Care for the Elderly) model, and as a value-based provider working in partnership with local health plans.

We are building a mission-driven team that shares our passion for redefining the way older adults experience care as they “age in place” in the community. This is a unique opportunity to take on one of our country’s most challenging healthcare problems and it is a unique moment in time to join us as we scale up to spread our mission across markets.

The Job

The Executive Director is responsible for serving as the senior operating leader for an assigned market, with full accountability for strategy, operations, culture, growth, participant experience, regulatory compliance, and financial performance. In close partnership with the Executive Medical Director, local leaders, and national leadership teams, this role drives the successful execution of the care model to achieve key clinical, quality, member experience, and business outcomes. The Executive Director leads and develops a high-performing market team, builds strong relationships with community and regulatory partners, and ensures the market operates in alignment with organizational goals and PACE requirements. This role is designed for a mission-driven, seasoned operating leader who brings experience leading integrated care operations, ideally in PACE or similar value-based care environments, and who is passionate about improving how care is delivered to vulnerable older adults while helping build capabilities that can scale across markets.

Other duties as assigned.

You Will

Serve as the executive operating leader for the market with full accountability for strategy, planning, operations, participant experience, compliance, and overall performance outcomes.

Partner with local and national clinical and operational leadership to drive high-quality care, strong clinical outcomes, participant satisfaction, operational excellence, and regulatory compliance.

Lead, develop, and retain a high-performing, diverse market team, including clinical and operational leaders, to support excellent care delivery and sustainable market success.

Establish, communicate, and model a clear operating strategy and culture vision that promotes accountability, engagement, service excellence, and alignment with organizational goals.

Collaborate with national leadership to implement, optimize, and sustain the infrastructure, systems, and operational processes needed to support all aspects of market performance.

Serve as the local expert in the care model and PACE regulatory requirements, ensuring the market maintains compliance and implements effective operational solutions to meet regulatory and organizational standards.

Oversee market financial performance, including budget development, resource planning, and ongoing monitoring of financial and operational results.

Represent the organization externally by building and maintaining effective relationships with providers, strategic partners, community-based organizations, and regulatory stakeholders.

Identify and advance opportunities to strengthen and adapt the care model to meet local community needs, improve participant outcomes, and leverage strategic partnerships.

Foster a culture of continuous learning, improvement, and operational ownership by leading with flexibility, sound judgment, and a service-oriented approach that supports participants, caregivers, and staff.

We seek Rebels who are curious about AI and its power to transform how we operate and serve our members.

Actively support the achievement of SCAN’s Vision and Goals.
