Haven Place in Lock Haven, Pennsylvania, is seeking a compassionate Licensed Practical Nurse to join our care team on an every-other-weekend schedule. You will provide high-quality patient care, administer medications, monitor vitals, and support daily activities to empower residents to live their best lives.

As an LPN, you will collaborate with interdisciplinary teams to develop care plans, report changes to nursing staff and physicians, and contribute to quality improvement initiatives.

Qualifications

• Graduate of an accredited LPN program.
• Current licensure as a Licensed Practical Nurse in Pennsylvania.
• Strong communication and interpersonal skills.

Responsabilités

• Provide high-quality patient care, including administering medications, taking vital signs, and assisting with daily living activities.
• Develop and implement individualized care plans in collaboration with interdisciplinary teams.
• Monitor and report changes in patient condition to nursing staff and physicians.
