Job summary

Home Care Network, Inc. in Granger, Indiana, is seeking a Respite Skilled Nurse (LPN or RN) to join our in-home care team. This part-time role covers weekday evenings (Tue, Thu, Fri 4 or 5pm–11pm) and Saturdays (12pm–8pm), with total ~25–30 hours per week. Full-time hours may be available if needed while caring for the current client.

The patient is a young adult female; you’ll provide ADL support, medication administration, and closely follow the care plan under an RN Case Manager.

Qualifications

• Graduate of an accredited nursing program and Indiana RN/LPN license.
• At least 1 year of paid in-home care experience preferred.
• CPR and First Aid certifications required or willingness to obtain.
• Strong communication, attendance and dependability.

Responsibilities

• Provide skilled nursing visits under RN supervision per care plan.
• Administer medications and monitor client condition.
• Document accurately and communicate changes to RN Case Manager.
• Ensure safety and HIPAA compliance in home environment.
• Collaborate with family and healthcare providers to coordinate care.
