Qualifications

Responsibilities

• Lead groups and individual sessions.
• Perform assessments in EHR and NJSAMS.
• Collaborate with clients to develop and maintain individualized treatment plans.
• Develop and monitor discharge/transfer plans to appropriate levels of care.
• Document all services in the EMR and on-license-reimbursement intervals.
• Work with the interdisciplinary team.
