Job summary

You will collaborate with an interdisciplinary team to ensure seamless transitions and maintain documentation for progress and treatment plans, while contributing to safety and excellence in care.

Responsibilities

• Conduct patient assessments, develop care plans, and implement interventions to promote patient health and wellness.
• Collaborate with the healthcare team to ensure seamless transitions and coordination of care.
• Provide direct patient care, including wound care, medication administration, and treatment administration.
• Document patient information, progress notes, and treatment plans accurately and timely.
• Participate in quality improvement initiatives and contribute to a culture of safety and excellence.
