Job summary

Qualifications

• Current California RN License.
• BSN or related degree preferred.
• 3–5 years acute care or home health experience; prior experience in Quality Improvement, Case Management, Discharge Planning or Utilization Management.

Responsibilities

• Independently manage a specific caseload and coordinate discharge plans.
• Collaborate with the health care team, patient, and family to achieve outcomes.
• Monitor utilization and review for medical necessity and reimbursement.
