Qualifications

• Bachelor’s degree in Human Services or related field is required.
• A valid driver’s license with acceptable driving record required.
• Johnson County Government requires reference/background screening for all positions.
• One year of related work experience is preferred.
• Education/Experience substitutions may apply; education may substitute for experience.

Responsibilities

• Conducts assessments of individuals' background and applicable circumstances to identify needs and/or determine service eligibility.
• Develops plans to address the needs of individuals and coordinates resources with other providers.
• Provides services for a designated caseload in a community-based setting and facilitates support groups.
• Completes documentation and evaluates progress according to measurable goals.
