Job summary

Augusta Health and Rehabilitation in Augusta, WI is seeking a compassionate LPN or RN to join our nursing team on the PM shift. This full-time role offers competitive pay: LPN $25.50/hr; RN $34.50/hr, with every other weekend and rotating holidays. We are a small, community-focused campus dedicated to resident-centered care.

As part of a close-knit team, you will provide direct nursing care, administer medications, document care, and work with CNAs to support residents and their families.

Qualifications

• Current Wisconsin LPN or RN license in good standing.
• Dependable and compassionate approach to resident care.
• Strong communication and teamwork skills.
• Ability to work independently while knowing when to ask for support.

Responsibilities

• Providing direct nursing care and monitoring residents' needs.
• Administering medications and treatments.
• Completing accurate and timely documentation.
• Communicating changes in condition to the appropriate members of the care team.
• Working collaboratively with CNAs and other nursing staff.
• Building positive relationships with residents and families.
• Helping create a safe, respectful, and comfortable environment for the people who call AHR home.
