Responsibilities

• Assist in admitting, discharging and transferring to ensure patient safety.
• Provide supportive, respectful care to adult clients and assist them in meeting treatment goals.
• Participate in case management, team meetings, and coordination with other service providers.
• Serve as liaison to inpatient facilities, case managers, psychiatrists, therapists, and families to maintain communication.
• Contribute to treatment planning and report issues to the Nurse during shifts.
• Document observed symptoms and changes in patient safety status.
