Job summary

cTrace Solutions, Inc. is seeking an experienced Licensed Vocational Nurse (LVN) for our client in San Angelo, TX. The LVN will provide high-quality nursing care across medical offices, assisted living facilities, and IDD centers, collaborating with healthcare professionals to optimize patient outcomes.

Salary: $40 per hour (DOE) with a $200 sign-on bonus. Shifts are 4 PM–12 AM, five days a week with every 3rd weekend off. Start dates can be flexible with 5 days prior processing.

Qualifications

• Licensed to practice as a Texas LVN or reciprocal per NLC.
• Ability to communicate both orally and in writing.
• Demonstrated computer & electronic health record skills preferred.
• Experience with IDD and psych preferred.

Responsibilities

• Administers medications and treatments orally, rectally, internally, or by injection. Performs nursing procedures per facility/state rules after competency demonstration; documents observations.
• Communicates information verbally and in writing to team members; documents per policy.
• Responds to emergencies to ensure safety; follows infection control procedures.
• Monitors patients for treatments and procedures.
• Collects specimens for lab tests and conducts screenings as requested after competency is documented.
• Provides education to individuals with specific health goals.
• LVN II assists others in turning/repositioning patients to administer treatments.
• Ensures supplies are available for patient care.
• Manages patient medical records in EHR or paper chart.
