Job summary

Pacific Coast Post Acute in Salinas, CA is hiring a Licensed Vocational Nurse (LVN) for a Full-Time PM shift. The LVN will provide direct nursing care, administer medications, monitor residents, and communicate changes to the team to support safe, compassionate care.

Qualifications

• Current and active California LVN license.
• Minimum of 2 years of SNF experience preferred.
• BLS/CPR certification preferred.
• Strong communication, organizational, and critical-thinking skills.

Responsibilities

• Provide direct nursing care in accordance with physician orders, care plans, and facility policies.
• Administer medications, treatments, and nursing procedures safely and accurately.
• Monitor residents' conditions and promptly communicate significant changes to the healthcare team.
• Complete accurate and timely documentation of resident care and clinical observations.
• Collaborate with physicians, therapists, nurses, and interdisciplinary team members to promote positive resident outcomes.
• Maintain resident comfort, dignity, safety, and well-being at all times.
