Windsor Nursing and Rehabilitation Center of Harlingen in Harlingen, TX is seeking an LVN Charge Nurse for a PRN 2pm-10pm shift to help provide high-quality resident care and coordinate with the medical team.

Requirements include completion of an LVN program and an active Texas LVN license; experience in a skilled nursing facility is preferred. You will administer medications, participate in rounds, document care, and supervise aides to support resident well-being.

Qualifications

• Has completed an LVN program at an accredited vocational school or college.
• Active Licensed Vocational Nurse license in Texas in good standing.
• Experience in a skilled nursing facility preferred.

Responsibilities

• Observe and report resident condition changes during rounds.
• Ensure care plans are implemented as directed.
• Administer medications as prescribed.
• Document physician visits and rounds in clinical records.
• Perform narcotic reconciliation with shifts.
• Identify changes in residents' condition and act per policies.
• Oversee nursing aides and supervision.
• Complete Incident/Accident Reports as required.
• Discharge residents per facility policy.
• Complete Nursing Assessments before physician calls for changes.
