Our Lady of Mercy Life Center, part of St. Peter's Health Partners in New York, is seeking a Full-Time Evening LPN to join our Sub-Acute Rehabilitation unit.

You will provide skilled nursing care, administer medications, and contribute to individualized care plans in a supportive, collaborative environment. We offer strong onboarding, flexible evening scheduling, tuition assistance, and opportunities for professional growth for both new graduates and experienced LPNs.

Qualifikationen

• Current New York State Licensed Practical Nurse (LPN) license.
• Graduate of an accredited Practical Nursing program.
• Strong clinical assessment and communication skills.
• Compassion, teamwork, and commitment to resident-centered care.
• Long-term care, rehabilitation, or skilled nursing experience preferred; new graduates encouraged to apply.

Aufgaben

• Provide skilled nursing care to residents in our Sub-Acute Rehabilitation unit.
• Administer medications and treatments per physician orders and nursing standards.
• Conduct resident assessments and report changes to the RN Supervisor and healthcare team.
• Assist in developing, implementing, and updating individualized care plans.
• Educate residents and families about treatments, recovery goals, and ongoing care needs.
• Collaborate with interdisciplinary team members to support quality outcomes and rehabilitation.
