The Center at Blue Hills is seeking a dedicated LPN or Registered Nurse for the Evening Shift to join our compassionate care team. You will provide nursing care to residents, administer treatments, and collaborate with an interdisciplinary team to ensure high-quality services and resident well-being.

Responsibilities include medication administration, care planning participation, and accurate documentation, with a focus on clear communication with residents and families.

Qualifications

• Must be licensed LPN or RN to practice in Massachusetts.
• Experience in long-term care or similar settings preferred.
• Strong communication and documentation skills.

Responsibilities

• Provide nursing care to residents under supervision of Nurse Manager and in collaboration with other healthcare professionals.
• Administer medications, treatments, and therapies as ordered by physicians and per established protocols.
• Monitor and report resident conditions, concerns, and changes to the Nurse Manager and team.
• Participate in care planning and coordination to ensure individualized care.
• Maintain accurate and timely records of resident care and services.
• Communicate effectively with residents, families, and interdisciplinary team members to ensure seamless transitions and optimal care.
