Job summary

Catholic Health's Father Baker Manor Sub Acute is seeking a Licensed Practical Nurse for evening shifts (2:00 PM–10:00 PM) with weekend and holiday rotation. This full-time position requires state licensure and prior long-term care exposure.

You will administer nursing care under a registered nurse, coordinate with CNAs, and follow the resident care plan to support comfort and safety of residents.

Qualifications

• High School diploma or GED.
• Graduate from an accredited school of practical nursing.
• Currently registered in New York State as a Licensed Practical Nurse.

Responsibilities

• The Licensed Practical Nurse administers nursing care to designated residents under the direction of a registered nurse.
• Coordinates and supports the activities of the certified nurse assistants and actively participates in the direct care of the resident, by following the plan of care.

Education
