Job summary

Masonicare at Ashlar Village in Wallingford, CT is seeking an LPN for the evening shift (24 hrs/week, EOW). Under SALSA supervision, the LPN provides nursing care, supports resident services plans, and collaborates with the health care team to ensure safe medication management and resident education.

Graduate of an accredited School of Nursing with 1–2 years of clinical hospital experience is preferred; current LPN license and CPR certification are required.

Qualifications

• Graduate of accredited School of Nursing.
• 1-2 years clinical experience in a hospital or similar health care setting preferred.
• Current LPN license; CPR certification.

Responsibilities

• Contributes to resident care and service plans through data collection and care planning meetings.
• Communicates with residents, families, and health care team to meet resident needs.
• Provides education, supervision and counseling to residents and families on AL procedures.
• Documents information accurately in the client service record in a timely manner.
• Oversees medication management under supervision and in collaboration with physicians and pharmacists.
• Performs other duties as assigned.
