Qualifications

• 1-2 years clinical experience in hospital or similar setting preferred.
• Current LPN license; CPR certification.

Responsibilities

• Contributes to resident care and service plans by collecting age- and ability-based data and participating in care planning meetings.
• Communicates and collaborates with residents, families and health care team to meet resident needs.
• Provides education, supervision and counseling to resident and family regarding AL procedures and other care needs.
• Documents accurate and timely information in the client service record.
• Conducts supervisory visits of resident care assistants and others as required.
• Participates in educational programs and workshops to enhance professional skills.
• Oversees medication management under supervision of SALSA/RN designee and in collaboration with physicians and outside agencies.
• Performs other duties as assigned.
