Responsibilities include medication administration, following care plans, documentation, and coordinating with an interdisciplinary team to deliver safe, person-centered care.

Qualifications

• Active Rhode Island LPN license.
• Valid BLS/CPR certification.
• Ability to manage multiple patients in a fast-paced setting.
• Strong clinical judgment and communication skills.

Responsibilities

• Provide patient care in accordance with established standards and written care plans.
• Follow individualized care plans developed by the Nurse Manager and interdisciplinary team.
• Administer medications and prescribed treatments per plan and facility policies.
• Monitor residents, report changes promptly, and respond to clinical needs.
• Maintain accurate documentation per physician orders and regulatory standards.
• Support a safe and therapeutic environment for residents and families.
