Job summary

Heritage Hills Rehabilitation & Healthcare Center in Rhode Island is seeking a part-time LPN to join our compassionate care team. The role includes shift 3-11 with daily pay options and a competitive hourly rate between $31 and $40, all-inclusive.

The LPN will provide patient-centered care, administer medications, and contribute to a safe, supportive environment. Qualified candidates hold an active RI LPN license and BLS CPR certification.

Qualifications

• Current, unencumbered LPN license in Rhode Island.
• Valid BLS Certification (CPR) required.
• Ability to care for and manage multiple patients in a fast-paced healthcare environment.
• Strong clinical judgment, communication skills, and attention to detail.

Responsibilities

• Provide patient care in accordance with the plan of care and physician orders.
• Follow and implement individualized care plans developed by the Nurse Manager and interdisciplinary team.
• Perform medication administration and prescribed treatments per the plan of care and facility policies.
• Monitor residents’ conditions, report changes promptly to the supervising nurse or physician, and respond to clinical needs.
• Maintain accurate, timely documentation in accordance with orders and regulatory standards.
• Support a safe, respectful, and therapeutic environment for residents and families.
