Qualifications

• Must be licensed as a Licensed Practical Nurse in the state of practice.
• Graduation from an accredited LPN nursing program.
• Long term care or sub-acute care experience preferred.

Responsibilities

• Provide professional nursing care for residents in assisted living under supervision.
• Assess residents and administer medications or treatments as ordered.
• Document resident interactions and care outcomes, and communicate with the care team.
• Maintain confidentiality and support an interdisciplinary care team.
• Counsel residents and families on prevention and treatment plans.
