Aliya Healthcare is seeking a dedicated Licensed Practical Nurse (LPN) to join our compassionate care team at our skilled nursing facility. As an LPN, you will provide total nursing care to residents on your unit and help supervise team members to ensure high-quality care.

You will administer medications, participate in care planning with the healthcare team, educate residents and families, and act as a charge nurse to support resident care decisions while upholding facility policies.

Qualifications

• Must have active LPN license in Illinois.
• Graduation from an accredited nursing program.
• No prior abuse convictions.
• Strong communication and teamwork skills.
• Ability to follow policies and maintain safety.
• Able to care for diverse resident populations.
• Physically able to perform nursing duties.

Responsibilities

• Provide total nursing care to residents on the assigned unit.
• Participate in developing and implementing individualized care plans.
• Administer medications and treatments as prescribed and evaluate effectiveness.
• Maintain an environment that promotes residents' well-being.
• Educate residents and families on care plans and procedures.
• Monitor team member performance and assist with development and evaluation.
• Delegate and ensure completion of assigned nursing tasks to team members.
• Stay current with facility policies and ongoing nursing education.
• Act as a charge nurse and support resident care decisions.
• Follow all facility safety policies and procedures.
