Qualifications

• Must hold an Illinois LPN license and graduate from an accredited nursing program.
• Ability to administer medications and follow care plans.
• Strong communication and teamwork required.
• Ability to work on PM shift and handle diverse resident needs.

Responsibilities

• Provide total nursing care to residents on the unit during the shift.
• Develop and implement individualized care plans with the healthcare team.
• Administer medications and treatments as prescribed and evaluate effectiveness.
• Maintain an environment that promotes residents' physical, social, and psychological well-being.
• Educate residents and families on care plans and medical procedures.
• Monitor team member performance and assist with development and evaluation.
• Delegate and ensure completion of assigned nursing tasks to team members.
• Stay current with facility policies and ongoing nursing education.
• Recognize and respond to significant changes in resident conditions.
• Document care, responses, and relevant information per guidelines.
• Act as a charge nurse: assist with questions and support care decisions.
• Follow all facility safety policies and procedures.
