THRYVE Healthcare in Crestwood, IL is seeking a Licensed Practical Nurse (LPN) to join our skilled nursing facility on the night shift. You will provide total nursing care, assist in care planning, administer medications, and supervise team members to ensure high-quality resident care.

The ideal candidate has an active IL LPN license, graduates from an accredited program, and strong communication and teamwork skills.

Qualifications

• Graduation from an accredited nursing education program.
• Active Licensed Practical Nurse (LPN) license in Illinois.
• No prior abuse convictions.
• Strong communication and teamwork skills.
• Ability to follow facility policies and deliver empathetic care.
• Physically able to perform nursing duties with or without accommodations.

Responsibilities

• Provide total nursing care to residents on the assigned unit during the shift as a Licensed Practical Nurse (LPN).
• Participate in developing and implementing individualized care plans alongside the healthcare team.
• Administer medications and treatments as prescribed and evaluate effectiveness regularly.
• Maintain an environment that promotes residents’ physical, social, and psychological well-being.
• Educate residents and families on care plans and medical procedures.
• Monitor team member performance and assist with development, evaluation, and corrective action if needed.
• Delegate and ensure completion of assigned nursing tasks to team members.
• Stay current with facility policies, procedures, and ongoing nursing education.
• Recognize and act on significant changes in resident conditions.
• Document care, responses, and relevant information per facility guidelines.
• Act as a charge nurse: answer questions, resolve issues, and support resident care decisions.
• Follow all facility safety policies and procedures.
