Warren Nursing & Rehab is seeking an Evening Shift LPN to provide compassionate, high-quality patient care for our residents and to work closely with the interdisciplinary team to support rehabilitation and overall well-being.

Responsibilities include direct patient care, medication administration, care plan development, vital signs monitoring, assist with transfers, documentation of care, and participation in care conferences to ensure seamless continuity of care.

Responsibilities

• Provide direct patient care to residents with diverse needs, including rehabilitation goals
• Develop and implement personalized care plans in collaboration with the interdisciplinary team
• Administer medications, treatments, and therapies as ordered by physicians
• Monitor and report vital signs and medical changes to the healthcare team
• Assist with transfers and mobility, offering emotional support and companionship
• Maintain accurate records of patient care, progress, and outcomes
• Participate in care conferences and team meetings to discuss patient care and progress
• Collaborate with other healthcare professionals to ensure seamless transitions and continuity of care
