Job summary

The Center for Living & Rehabilitation in Bennington, VT is seeking a dedicated Licensed Practical Nurse (LPN) to join our evening shift team. You will provide high-quality, patient-centered care to our residents as part of an interdisciplinary team, promoting a positive and supportive environment.

Responsibilities include administering medications, wound care, taking vital signs, developing care plans with the team, documenting progress, and ensuring continuity of care during handovers.

Qualifications

• Current LPN licensure in the State of Vermont
• Strong communication and interpersonal skills

Responsibilities

• Provide direct patient care, including administering medications, wound care, and performing vital signs
• Develop and implement individualized care plans in collaboration with interdisciplinary team members

• Participate in shift reports and handovers to ensure continuity of care
• Collaborate with other healthcare professionals to promote a positive and respectful environment
• Maintain accurate and up-to-date patient records
