Job summary

Heritage House in Troy, NY is seeking a Licensed Practical Nurse for a full-time evening role (3pm-11pm) to join our skilled nursing team. You will provide nursing care under the direction of a Registered Nurse and support treatments and procedures per the established plan of care.

The position requires a current NY LPN license and a commitment to compassionate, person-centered care. Pay ranges from $24.00 to $30.10 per hour, with pay based on experience, skills and education; exempt positions

Qualifications

Current NY LPN license is required.
Evening shift availability (3pm-11pm) is required by the role.
Experience in skilled nursing settings is preferred.

Responsibilities

Administers nursing care to residents under the direction of an RN and carries out selected duties within the established nursing plan of care.
In collaboration with the RN, collects data about residents and prioritizes care based on identified needs and goals.
Contributes to the plan of care through effective communication with the RN and healthcare team.
Administers medications and monitors IV therapy as ordered and according to policies and standards.
