Qualifications

• LPN license in NY is required.
• Proficient in English, reading and following instructions.
• BLS certification within 90 days of hire.
• Ability to work evenings and weekends.
• Strong communication and interpersonal skills.
• Familiarity with electronic medical records.

Responsibilities

• Provide direct patient care including medication administration and wound care.
• Monitor residents and report changes to the nursing team and physicians.
• Assist with transfers, ambulation and mobility exercises.
• Ensure resident safety by identifying hazards and initiating corrective actions.
• Maintain accurate documentation of resident care and treatment.
• Participate in quality improvement initiatives to improve outcomes.
• Collaborate with other departments to ensure seamless care delivery.
