Job summary

American River Hospice Services is seeking an Evening Licensed Vocational Nurse (LVN) to join our compassionate hospice team in California. The LVN will provide basic nursing care, monitor patient health, and document medical histories with strong attention to detail.

Qualifications

Must hold a valid LVN license in good standing with the state board.
Minimum 2–3 years of hospice experience preferred.
Strong communication skills and ability to work collaboratively with healthcare professionals.
Proficiency with electronic medical records (EMR) systems.
Excellent attention to detail and problem-solving abilities.

Responsibilities

Administer medications, immunizations, and treatments as prescribed by physicians or providers.
Monitor and document patients' vital signs and medical history.
Develop and maintain patient care plans with input from physicians and other clinicians.
Educate patients and families about health conditions, medications, and treatment options.
Collaborate with physicians, nurses, and other professionals to implement care plans.
Follow infection control procedures.
Document patient care and progress in EMR.
