Job summary

SSM Health St. Agnes in Wisconsin is seeking a Part-time Certified Nursing Assistant for the Hospice Home of Hope at Fond du Lac. Schedule: 14 hours per week, evenings (4:00 PM–8:00 PM) with every other weekend rotation.

Requires Wisconsin CNA certification and completion of approved nurse aide training. Prior experience in long-term care is preferred but not required. You will assist with a range of nursing services, document patient data, and collaborate with the RN to ensure compassionate,

Qualifications

• Completion of an approved nurse aide training program and passing grade on state of work location required exam.
• No experience required.
• Wisconsin CNA certification preferred or required depending on location.

Responsibilities

• Assists with nursing care services in hospice house.
• Documents data for RN assessment and communicates changes in patient condition.
• Provides basic patient care: bathing, feeding, hygiene, transfers, and vital signs.
• Prepares patients for discharge and accompanies them as needed.
• Maintains supplies and equipment and supports clerical duties as directed.
