Job summary

Trinity Health Senior Communities is seeking a Certified Nursing Assistant for the Evening Shift to provide routine daily nursing care per each resident's plan. You will assist with daily living activities and support admission, transfer, and discharge processes.

Responsibilities include accurate documentation in Care Tracker, maintaining privacy, and ensuring safety. The role requires CNA certification, strong customer service, and the ability to work effectively in a team within a geriatric

Qualifications

• High School Diploma or equivalent required.
• Current, unencumbered CNA certification to practice in Indiana.
• Minimum of one year in healthcare preferred; geriatrics/long-term care preferred.
• Strong customer service, verbal and written communication skills.
• Ability to adapt to change and work in a fast-paced, customer-driven environment.
• Interpersonal skills for cross-functional teamwork and resident interactions.

Responsibilities

• Provide routine daily nursing care per resident care plan.
• Assist residents with bathing, dressing, grooming, feeding and mobility.
• Support admission, transfer and discharge processes.
• Document data accurately in Care Tracker and communicate changes to supervisors.
• Maintain privacy and safety, following policies and procedures.
