Job summary

Qualifications

Responsibilities

• Assist with transferring patients, lifting, and moving them safely.
• Monitor and report changes in patient condition to nursing staff.
• Support patients with daily living activities, such as mealtime and toileting.
• Participate in patient care conferences and develop individualized care plans.
• Collaborate with nursing staff to ensure seamless care delivery.
• Maintain accurate and up-to-date patient records.
