Job summary

The GreenFields Continuing Care Community in New York seeks an Enhanced Care Partner (Home Health Aide) to provide compassionate, person-centered onsite support to residents living with dementia or physical challenges. You will assist with meals, housekeeping, activities, personal care, and safety, following individualized care plans, while promoting dignity and independence.

This role requires a current HHA certification and at least one year of elder care experience; high school diploma

Qualifications

• Must be currently certified as a Home Health Aide.
• Minimum of one year of work experience with the elderly; memory care preferred.
• High School diploma or two years of related work experience.
• Ability to read, write, speak, and understand English.
• Ability to move and lift up to 50 pounds; physical demands of the role.

Responsibilities

• Provide meal preparation and dining assistance per recipe book.
• Clean resident rooms and common areas; maintain tidiness of kitchen and spaces.
• Offer recreational activities and engage residents under supervision.
• Assist with bathing, grooming, transfers, and incontinence care as needed.
• Perform personal laundry and assist with ambulation and positioning.
• Document activities and communicate changes to nursing staff.
• Ensure safety and respond to call lights promptly.
