Job summary

Northern Path Recovery Center in Fort Wayne, IN is seeking a Licensed Counselor - Addiction Treatment (Evenings) to join our onsite team.

The Counselor Two provides assessment, counseling, case management and crisis intervention to support clients’ recovery and coordinates with families and referral sources. Must hold a master’s degree and valid licensure and have 1+ year post-graduate experience in addictions or counseling.

Qualifications

• Master’s degree in social services, addictions, mental health or closely related health services field with 1+ years of post-Masters degree professional experience in the addictions, therapy or counseling field.
• Must have current and valid licensure or certifications as a social worker, professional counselor, therapist or addiction counselor per state agency.

Responsibilities

• Conduct client assessments focused on identifying client history, current presentation, problem areas, and ongoing clinical need for treatment and risk assessment.
• Conduct ongoing assessments of chemical dependence services required by the treatment program.
• Prepare and carry out client initial treatment planning and treatment plan progress reviews as assigned and documents as required.
• Lead assigned group counseling, individual or family sessions and implement evidence-supported treatment modalities as assigned and documents as required.
• Provide client and family education relative to substance abuse/psychiatric problems.
• Ensure all documentation regarding client care, treatment and incidents is completed timely and in a clear, concise manner.
• Provide crisis intervention and counseling to clients, as needed.
• Plan for and complete required documentation of aftercare for assigned clients.
• Act as a liaison with insurance carriers, as requested.
• Act as a liaison between referral sources and clients.
• Provide case management duties for clients, ensuring individualized quality care.
• Conduct educational workshops both within the Hospital and in the community as required.
• Serve as a liaison with self-help groups and EAP professionals.
• Evaluates the needs of the client to determine the appropriate level of care and determines if referrals to other programs or facilities is needed.
