Job summary

Kadima Health and Nursing at Pottstown is seeking an Activity Assistant to join our team for a scheduled 4pm–8pm shift. You will plan, monitor, and participate in unit activities, guiding residents through individual and group activities while ensuring safe use of all equipment.

You will also document participation, assessments, and care plans, and work with activities staff to build strong relationships with agencies and volunteers.

Qualifications

• Must have a high school diploma or equivalent.

Responsibilities

• Plans and monitors unit activities, content of programs and independent activities.
• Implements individual and group activities; supervise residents' use of all equipment and materials and takes necessary precautions in implementing the program.
• Oversee documentation of resident participation assessment, MDS, care plan and progress on the appropriate forms
• Works with activities staff to establish and maintain effective working relationships with agencies and volunteer groups.
• Other duties as assigned by the supervisor
