Job summary

Lancesoft INC is seeking a Licensed Clinical Social Worker (LCSW/LMSW) for our Emergency Department in Delaware. This full-time evening or night role focuses on care coordination, crisis intervention, and discharge planning to support patients and families.

The ideal candidate holds a MSW or Master of Social Services, Delaware LCSW/LMSW, at least 2 years in acute care, and preferably maternal-child experience; CCM certification is a plus.

Qualifications

• MSW or Master of Social Services from an accredited institution.
• LCSW or LMSW license in Delaware prior to start date.
• Minimum 2 years in acute care social work; Emergency Department experience preferred.
• Maternal-Child experience preferred for candidates in that specialty.
• CCM certification preferred within 18 months of eligibility.

Responsibilities

• Collaborate within the Care Management Triad Team Model: RN Case Manager, Social Worker, Utilization Management.
• Conduct psycho-social assessments and interventions.
• Support patients and families during crisis situations.
• Coordinate discharge planning and referrals to community resources.
• Work closely with interdisciplinary teams to ensure continuity of care.
