Responsibilities

• Performs plan of care intervention, including medication administration and clinical treatments.
• Documents patient care given.
• Monitors, records, and communicates patient condition to care team, physician, patient, and family.
• Assists patients with activities of daily living (ADLs) and nutrition.
• Collaborates across disciplines to coordinate patient care, including transfers and discharges.
• Advocates for the physical and emotional well being of the patient.
• Engages with families to support patient recovery and understanding of illness.
