Job summary

Qualifications

• HLFS certification required.
• ACLS certification required.

Responsibilities

• Collects data, analyzes diagnosis and care, plans interventions, implements the plan and coordinates care.
• Collaborates with the patient, family and healthcare team to provide safe, humane care and educate patients and families.
• Participates in discharge planning to ensure continuity of care.
• Delegates and coordinates duties of healthcare team members.
